Matching Incentive Program (MIP) Rules
Employee Choice: Applies to donations made to any US-based 501(c)(3) non-profit organizations (except accredited colleges or universities - see "Higher Education Choice" below) or K-12 public or private schools.
TOTAL ANNUAL LIMIT PER EMPLOYEE: $1,000 per year matched 1:1
- All US-Based active full-time and part-time employees are eligible; retirees are not eligible
- Donor must be an active full-time or part-time employee when donation is made, registered, and confirmed by nonprofit organization.
- Minimum gift eligible for match: $25.00
- In order to be eligible for a MIP match, employee/retiree donations must be in the form of a personal tax-deductible contribution of $25 or more from the employee/retiree's own assets, paid directly to the eligible 501(c)(3) organization or NCES institution. Donations must be current contributions from the employee/retiree's own assets, and must be made, not merely pledged. A matching gift cannot be used to satisfy a personal pledge (e.g. a pledge of $2,000 cannot be satisfied by $1,000 from the employee and $1,000 from the Foundation match). Donations can be in the form of one of the following:
- personal check
- money order
- credit card payment
- transfer of stock certificate
- Donations must be given and registered online within the current calendar year (i.e. donations given between 01/01/10 and 12/31/10 must be registered prior to midnight ET on 12/31/10)
- In order to receive a match, the recipient nonprofit organization must verify receipt of the gift online no later than March 31st of the following calendar year (i.e. gifts registered in 2010 must be confirmed by 03/31/11)
- The Verizon Foundation/Verizon Foundation Customer Service Center is not permitted to register a donation on behalf of an employee/retiree.
- Employees are not permitted to confirm/verify their own MIP gift transactions on behalf of a nonprofit organization
- Employees are not permitted to confirm/verify MIP gift transactions made by another Verizon employee on behalf of a nonprofit organization
- Employees' family members* are not permitted to verify/confirm the employees' matching gift donation on behalf of a nonprofit organization.
- Employees are not permitted to request a MIP grant on behalf of organizations where they or an immediate member of their family* receive any direct or indirect commercial or financial benefit as a result of their donations to those organizations - including but not limited to memberships, dues, tuition, tickets.
- The benefitting nonprofit organization/school may not enter a matching gift donation on behalf of an employee or retiree
- The Verizon Foundation will not consider a MIP grant to be eligible for payout if there is any discrepancy between the donation dollar amount entered by the donor and the donation dollar amount verified/confirmed by the nonprofit.
- Matching Grant funds from the Verizon Foundation may not be used to fulfill membership dues, reach donation thresholds that provide any personal benefit or gain, satisfy a personal pledge or the like.
Higher Education Choice: Applies to donations made directly to US accredited colleges and universities only

- All US-Based active full-time and part-time employees and eligible retirees* (*Please Note: retirees of Verizon Wireless and MCI are not eligible to participate in MIP)
- Donor must be an active full-time or part-time employee or eligible retiree when donation is made, registered, and confirmed by nonprofit organization.
- Minimum gift eligible for match: $25.00
- Donations must be given and registered online within the current calendar year (i.e. donations given between 01/01/10 and 12/31/10 must be registered prior to midnight ET on 12/31/10)
- In order to receive a match, the recipient nonprofit organization must verify receipt of the gift online no later than March 31st of the following calendar year (i.e. gifts registered in 2010 must be confirmed by 03/31/11)

- The benefitting nonprofit organization/school may not enter a matching gift donation on behalf of an employee or retiree
- Matching Grant funds from the Verizon Foundation may not be used to fulfill membership dues, reach donation thresholds that provide any personal benefit or gain, satisfy a personal pledge or the like.
*Immediate family is defined as:
- The employee's spouse or same-sex domestic partner
- The employee's/employee's spouse's/employee's same-sex domestic partner's child, son or daughter-in-law, grandchild, or great-grandchild
- The employee's/employee's spouse's/employee's same-sex domestic partner's step child, foster child or child for whom the individual is the legal guardian
- The employee's/employee's spouse's/employee's same-sex domestic partner's parent, stepparent, grandparent, step-grandparent or great-grandparent
- The employee's brother/sister, employee's brother-in-law/sister-in-law or same-sex domestic partner's brother/sister (including half, adopted and step)
- The employee's/employee's spouse's/employee's same-sex domestic partner's aunt, uncle, niece or nephew
- The Verizon Foundation will not consider a MIP grant to be eligible for payout if there is any discrepancy between the donation dollar amount entered by the donor and the donation dollar amount verified/confirmed by the nonprofit.
The Verizon Foundation cannot process any gift where there is a discrepancy between the gift amount entered by the donor and the amount confirmed by the nonprofit.
Please be sure to abide by all Matching Gifts Program rules. Only those application that adhere to the rules will be considered for approval and payout. All Matching Gift requests are subject to review by Verizon Internal Audit and Verizon Security. As stated in the code of Conduct, Verizon does not tolerate falsification or improper alteration of records. Failure to comply with any provision of the Code or company policy is a serious violation, and may result in disciplinary action, up to and including termination of employment.
